Spravato® is the brand name for esketamine, a prescription nasal spray that affects the brain’s glutamate system. This differs from many conventional antidepressants, which primarily act on serotonin, norepinephrine, or dopamine.
The FDA has approved Spravato for adults with treatment-resistant depression, either as a standalone medication or with an oral antidepressant. It is also approved with an oral antidepressant for depressive symptoms in adults with major depressive disorder who are experiencing acute suicidal thoughts or behavior.
These approvals apply to major depressive disorder, not bipolar depression. Using Spravato for bipolar would therefore be considered an off-label application based on a clinician’s judgment, the available evidence, and the individual patient’s needs.

Bipolar disorder is characterized by episodes of depression as well as periods of mania or hypomania. Some individuals also experience mixed episodes, during which depressive symptoms occur alongside increased energy, agitation, impulsivity, or racing thoughts.

People with bipolar disorder were generally excluded from the pivotal trials that established Spravato’s FDA-approved uses. As a result, the evidence for bipolar depression is not as extensive as it is for treatment-resistant major depressive disorder.
Small observational studies and real-world reports have suggested that intranasal esketamine may reduce depressive symptoms in some people with treatment-resistant bipolar depression. However, these findings do not establish Spravato as a standard or universally appropriate bipolar treatment.

Depending on your needs, care may include psychiatric medication, sleep and routine stabilization, lifestyle support, or psychotherapy. Cognitive behavioral therapy may help patients recognize thought patterns, cope with depressive symptoms, and identify early warning signs of mood changes.
